If there are any other signs or symptoms associated with the pain?

The chief complaint?
If there was any trauma or factors that caused the pain?
If you have any secondary complaints?
If you are on medications?
If you have allergies?
The exact location and any radiation of the pain?
 
 
If there is any weakness associated with your pain?
If there is any loss of bowel or bladder control?
Past medical history which may be a factor?
Family history which may be significant?
Significant social factors as stress, diet, work or exercise habits?
  The above findings will play a role in the determination of the diagnosis.
